Admission Process

Every potential receipient of services is provided the appropriate information regarding specific eligibility requirements by the Program Director or his/her designee. Once eligibility is determined and the adminission is agreed upon, the following process will be followed:

Completion of all admission information which includes, but not limited to:

Basic information
Social history
Person Centered Plan (adults) / Introduction Information (preschool)
Individual Plan (temporary)
Review of Parent/Gaurdian responsibilities
Review of Participants responsibilties

This information will be complied by the designated individual for each service component.

The individual plan will be written to develop the specific goals and objectives of the service selected and will be tailed to each individual's needs. This document will be periodically reviewed and monitored to determine process. Meetings will be held, as determined by each program regulatory requirment, to review and discuss the effectiveness of the plan and to update the plan as needed.

All information is kept in accordance with the Health Insurance Portability and Accountability Act (HIPAA) of 1996. No identifiable information can be shared without prior authorization by the individual recipient or their legal guardian/representative. A release of information must be executed before information can be shared.
