Case Management assists people in gaining access to needed medical, social, educational and other services. These services are available through UCP to assist eligible recipients.

At United Cerebral Palsy, case managers meet with individuals and families to discuss specific needs. During the initial meeting, an assessment is completed and the options are discussed. The individual is provided with information necessary to identify appropriate services. Case management focuses on the DDS Home and Community Based Medicaid Waiver Program in efforts to help individuals live in the least restrictive environment while maximizing efforts to increase independence. Case managers monitor waiver services to ensure selected services and supports are meeting the individual needs of each recipient. Case Management services are designed to respond to changes in consumer needs and create working plans that adjust to individual goals.

United Cerebral Palsy strives to help all individuals with disabilities to live a “life without limits”, through the careful construction of an individualized plan of services and supports.

Case Management Services participates in the Alternative Community Services program statewide.
